We would like to welcome all of you to campus as we approach the beginning of another exciting year. In an effort to streamline the clearance process, you will need to complete several forms prior to participation. For your convenience, we have made our forms available online. The link below will direct you to these downloadable documents.
These documents must be completed prior to the beginning of every school year. These forms must be completely filled out, signed, and turned into the Athletic Staff before you will be cleared for participation. If all of these forms are not complete, you will not be cleared for participation until they are complete.
Every year, the athlete and/or parent or guardian must:

Complete and sign the Athletic Insurance Questionnaire

Provide a photocopy of the front and back of the athlete's valid insurance ID card

Read and sign the Informed Consent and Medical Waiver

Undergo a physcial examination by a physican
The completed and signed forms must be on file with the athletics department no later than August 1 for fall athletes and by September 1 for all other athletes.
Please Note: All SCAD student athletes will be required to show proof of primary insurance coverage. The student primary coverage must cover all intercollegiate sports related injuries. Any student-athlete that does not show proof of primary insurance coverage for all intercollegiate sports injuries will be required to:

Enroll in an individual accident or health plan of your choosing, that covers intercollegiate sports injuries; or

Enroll in the Uninsured Student Athlete Plan provided by for the SCAD Department of Athletics. Premiums for this coverage will be collected by SCAD from the student and then remitted to the insurance company for processing.
